Core Treatments

Expert care, tailored to you

Every treatment begins with a thorough assessment to fully understand your symptoms, lifestyle and medical background. From there, a personalised plan is built around your specific needs.

01

Osteopathy

Osteopathy is a regulated healthcare profession requiring a Master's degree and a minimum of 1,000 hours of supervised clinical training — covering anatomy, physiology, pathology, pharmacology, biomechanics and diagnosis. Practitioners must register with the General Osteopathic Council (GOsC) and meet ongoing continuing professional development requirements.

Each session begins with a detailed case history, followed by a physical assessment and hands-on treatment using techniques tailored to your presentation: joint manipulation, soft tissue therapy, myofascial release, visceral techniques, and cranial osteopathy. This holistic approach addresses current symptoms while building the longer-term resilience of your musculoskeletal system.

Dry Needling

Dry needling is a precise, evidence-informed technique used within osteopathic sessions to target myofascial trigger points — localised areas of muscle that have become hyper-irritable and refer pain. Fine, single-use sterile needles are inserted directly into the trigger point, provoking a brief local twitch response that signals muscle release and initiates the body's natural repair process.

A 2023 umbrella review of 36 systematic studies confirmed that dry needling produces clinically meaningful reductions in musculoskeletal pain compared to no treatment or sham intervention. At Nexus Clinic it is combined with osteopathy in the same session at no additional cost — treating the muscular and structural components of pain together.

The evidence

What the research says about osteopathy

Osteopathy is not a complementary therapy — it is a regulated healthcare profession underpinned by a growing body of high-quality clinical evidence. Here is what the science says.

General Osteopathic Council · UK Statutory Regulator

A protected title, regulated by law

Unlike many healthcare titles in the UK, "osteopath" is a protected title under the Osteopaths Act 1993. It is illegal to use the title without being registered with the GOsC. As of 2024, there are approximately 5,100 registered osteopaths in the UK, all of whom must complete a recognised degree programme of at least four years' full-time study covering anatomy, physiology, pathology, pharmacology, biomechanics and a minimum of 1,000 hours of supervised clinical practice — before they can register to practice. Registration also requires mandatory continuing professional development and professional indemnity insurance.

Journal of Bodywork & Movement Therapies · Overview of Systematic Reviews · 2025

27 systematic reviews confirm efficacy across multiple conditions

A 2025 overview of 27 systematic reviews published in the Journal of Bodywork & Movement Therapies (Zipp et al., Medical University of Graz) found that based on moderate-quality evidence, osteopathic treatment effectively reduces pain and improves physical function in adults with acute neck pain, chronic non-specific low back pain, pregnancy-related back pain, and chronic non-oncologic pain. This overview of overviews draws on RCT data across nine therapeutic indications and represents the strongest available synthesis of the current evidence base for osteopathy.

Clinically superior to sham — confirmed by GRADE evidence

A 2024 systematic review and meta-analysis published in Diseases (Alvarez-Bustins et al.) searched PubMed, PEDro, the Cochrane Library and Web of Science up to September 2024, applying the PEDro scale for methodological quality and GRADE for evidence certainty. By comparing osteopathic manipulative treatment directly against sham or placebo interventions — the most rigorous methodological design — the review found OMT to be clinically superior in reducing pain intensity and disability in patients with neck pain and low back pain. This sham-controlled design rules out non-specific effects and placebo response, isolating the genuine clinical contribution of osteopathic technique itself.

At Nexus Clinic

What happens in a session

Every appointment at Nexus Clinic begins with a thorough case history — exploring your symptoms, medical background, lifestyle and goals. This is followed by a physical assessment, combining postural observation with orthopaedic and neurological tests where appropriate. Treatment begins in the same session. Depending on your presentation, techniques may include high-velocity low-amplitude thrust techniques (HVLAs), articulation, myofascial release, visceral work, cranial techniques, and dry needling. Each plan is entirely individual — no two patients receive the same treatment.

Core techniques — and much more besides

Joint Manipulation (HVLA)
Short-lever, high-velocity, low-amplitude thrusts that restore joint mobility, reduce local muscle guarding and improve pain signals. The familiar "click" is cavitation — a gas bubble releasing in the joint fluid, not bones cracking.
Soft Tissue Therapy
Rhythmic stretching and pressure applied to muscles and fascia to reduce hypertonicity, improve local circulation and prepare tissue for deeper structural work.
Myofascial Release
Gentle, sustained pressure into connective tissue restrictions — working with the whole fascial network rather than isolated muscles. Particularly effective for chronic pain and postural dysfunction.
Articulation
Repetitive passive joint movement through its range of motion to restore fluid mechanics, reduce stiffness and stimulate synovial fluid production. Low-force and well-tolerated.
Cranial Osteopathy
Extremely gentle, subtle techniques applied to the skull and sacrum to address the craniosacral rhythm. Often used for headaches, jaw pain, stress-related tension and post-concussion presentations.
Visceral Osteopathy
Targeted assessment and treatment of the mobility of internal organs and their connective tissue attachments — influencing musculoskeletal function through the body's interconnected systems.

Dry needling explained

Precision treatment for muscle pain

Dry needling is frequently misunderstood or confused with acupuncture. Understanding how and why it works helps set realistic expectations — and explains why it is so effective when used as part of an integrated osteopathic approach.

What is dry needling?

Dry needling uses the same fine, sterile, single-use needles as acupuncture — but is rooted in Western anatomy and neuroscience rather than traditional Chinese medicine. The needle is inserted directly into a myofascial trigger point: a localised, hyperirritable spot within a taut band of skeletal muscle that causes a predictable pattern of referred pain when compressed or stimulated.

The term "dry" distinguishes the technique from "wet needling" — injections of local anaesthetic or corticosteroid. The needle itself is the therapeutic instrument, with no substance injected.

How does it work? The mechanism

When the needle reaches a trigger point, it typically provokes a brief involuntary muscle twitch — called a local twitch response (LTR). This twitch is a positive clinical sign: it indicates precise needle placement and is associated with the desensitisation of the trigger point. The proposed mechanisms include mechanical disruption of dysfunctional motor end-plate activity, modulation of nociceptors (pain-sensing nerve endings) and stimulation of the body's own endogenous analgesic systems via the release of enkephalins and other neurotransmitters.

The result is a reduction in local muscle tension, improved blood flow to previously ischaemic tissue, and a lasting decrease in referred pain.

What does the evidence say?

A 2023 umbrella review synthesising data from 36 systematic reviews — published in the Journal of Clinical Medicine by researchers at Ghent University — concluded that dry needling produces clinically meaningful reductions in musculoskeletal pain compared to no treatment or sham intervention across multiple body regions.

A separate systematic review published in Disability and Rehabilitation (2024, Tufts/Duke collaboration) found that dry needling produced similar outcomes to cortisone injections and platelet-rich plasma (PRP) for musculoskeletal pain disorders making it a clinically viable, non-pharmacological alternative for many patients.

Dry needling vs acupuncture — what is the difference?

Both techniques use the same type of needle, but they differ in their theoretical framework, target and intent. Acupuncture is grounded in Traditional Chinese Medicine, targeting specific meridian points to balance the flow of qi (life energy). Dry needling is grounded in Western musculoskeletal anatomy: needles are placed into identifiable anatomical structures — trigger points within specific muscles — based on physical examination findings rather than a meridian map.

At Nexus Clinic, dry needling is practised as an adjunct to osteopathic assessment and treatment. The trigger points targeted are identified during the same clinical examination that guides all manual therapy decisions — making it a seamless, integrated part of care rather than a separate modality.

Dry needling is included within the osteopathy appointment at no extra charge — it is part of the treatment, not an add-on.

Most patients experience immediate muscle release and reduced pain sensitivity. Mild localised soreness for 24–48 hours post-treatment is normal and indicates the tissue is responding.

Safety profile

Dry needling, when performed by a trained and regulated practitioner, is considered safe. Adverse events are typically mild and transient — localised soreness, occasional minor bruising, or brief light-headedness. Serious complications are rare. Single-use, sterile needles are used in every case. Contraindications include active infection at the needle site, blood-thinning medication (discussed at assessment), needle phobia and certain medical conditions — all screened during the initial case history.

I can't recommend Max highly enough. I came to him diagnosed with Thoracic Outlet Syndrome — incredibly debilitating, chronic pain affecting my day-to-day life. Prior to seeing Max, physiotherapy had made my symptoms even worse. From the first session I felt I was in safe, knowledgeable hands. After several sessions the improvement has been huge — the sensations that were once constant have significantly died down, and I've started enjoying my life again. Incredibly grateful.

FAQ

Common questions

Your initial appointment (45 mins) begins with a detailed case history — I'll ask about your symptoms, lifestyle and medical background. A thorough physical assessment follows, and hands-on treatment begins in the same session. I'll explain my findings clearly and outline a personalised treatment plan.
This varies depending on your condition. Acute issues often resolve in 2–4 sessions. Chronic or complex conditions may require more. I'll give you an honest estimate at your first appointment and reassess regularly — I never recommend more sessions than necessary.
Many private health insurers — including AXA, Bupa International, Vitality and Simply Health — cover osteopathy. It is ultimately your responsibility to confirm your entitlement to reimbursement with your insurer before booking. Nexus Clinic provides a detailed invoice containing all the information your insurer will need to process a claim — simply forward it to them directly.
Dry needling uses fine, sterile needles inserted into tight muscles to release trigger points and reduce pain. You may feel a brief twitch or dull ache — this is normal and positive. Most patients find it very tolerable and notice significant relief quickly.
